I think that Sirius had himself together much better in OotP than we often give him credit for. He was morose and moody, but on the whole, very rational in his interactions with others and in conversation. IIRC, Sirius says that Harry is less like James than he expected at one point only, and that is when Harry refuses to meet him in Hogsmeade. That's not a sign of confusing Harry with James. That's a blatant attempt to manipulate Harry into something by making him feel guilty. Not nice, perhaps, but not an indication of a deranged mind.
